Abstract
Background: We sought to determine if a brief simulation-based curriculum could improve cardiology fellows’ confidence in and ability to adequately perform transvenous pacemaker (TVP) insertion, programming, and troubleshooting. The aim was to raise the level of confidence and performance to that of senior fellows. Methods: We examined fellows' ability to perform the full TVP procedure in 2 parts: placing the central venous line (CVL) and inserting the TVP wire. The most experienced represented the control groups (2nd & 3rd years for CVL, 3rd years for TVP), and the least experienced the intervention groups (1st years for CVL, 1st & 2nd years for TVP). Intervention groups received simulation training, and all participants received CVL and TVP validated 30-point checklist performance assessments and a course survey. Results Pre and post simulation curriculum survey scores showed a statistically significant increase in fellows' rating of their confidence in TVP performance ability (p=0.002) and understanding (p=0.004). Assessment scores for the CVL and TVP portions were not significantly different between intervention and control groups. Conclusion: A simulation-based curriculum significantly improved cardiology fellows’ confidence and understanding of the complete TVP procedure and advanced their performance on both CVL and TVP assessments to a level not different from more experienced senior fellows. Fellows only required a 3 hour simulation course to demonstrate substantial improvements.
