Abstract
Introduction: ICONIC-LEAD(NCT06095115), ICONIC-ADVANCE1(NCT06143878), and ICONIC-ADVANCE2(NCT06220604) are evaluating the targeted oral peptide icotrokinra(ICO) for treatment of moderate-to-severe plaque psoriasis(PsO). Within each study, co-primary endpoints were met and participants achieved significantly higher rates of skin clearance with ICO vs placebo(PBO). This pooled analysis assessed skin response across baseline subgroups of interest. Methods: Participants ( ≥ 12 years LEAD; ≥ 18 years ADVANCE1&2) with moderate-to-severe plaque PsO (body surface area ≥ 10%; Psoriasis Area and Severity Index[PASI] ≥ 12; Investigator’s Global Assessment[IGA] ≥ 3) were randomized (2:1 LEAD; 2:1 ADVANCE1; 4:1 ADVANCE2) to once-daily oral ICO 200mg or PBO. This Week(W)16 analysis assessed proportion difference in IGA 0/1 response across sex, age (<18/18–44/45–64/ ≥ 65), body weight (BW; ≤ />90kg), body mass index (BMI; <25/25–29/ ≥ 30kg/m2), PASI (98.8kg) subgroups. Results: Pooled analyses included 1089 ICO- and 466 PBO-treated participants. ICO demonstrated higher IGA 0/1 response rates vs PBO at W16 (67.5% vs 9.2%); treatment effect (proportion difference: 57.9% [95%CI:53.8–61.6%]) was consistent across subgroups (range:51.8–64.2%). ICO also provided substantially greater PASI percentage improvements vs PBO at W16 (81.9% vs 25.8%); treatment effect (LSM difference: 56.1% [95%CI:52.8–59.4%]) was consistent across BMI and BW quartile subgroups (range:49.1–60.2%). Conclusion: In a large pooled-cohort of participants with moderate-to-severe plaque PsO, the robust effects of the targeted oral peptide ICO on skin clearance were consistent regardless of baseline PsO severity, sex, age, weight, or prior treatment.
