Abstract
The pathogenesis of hidradenitis suppurativa (HS) involves inflammation of the hair follicles, and treatment with laser hair reduction (LHR) that facilitates the destruction of follicle units is a viable treatment option. Current literature shows promising efficacy for LHR in HS patients, but no established best practices exist for this treatment modality. This study was conducted from January to May 2025 and utilized a modified Delphi consensus method to reach agreement on the best practices for treating HS with LHR. An 8-member steering committee of HS experts developed 32 statements regarding patient selection criteria, laser modality, concurrent therapy guidance, efficacy, and future directions. These statements were based on literature and extensive discussion among the committee. Members of the HSPlace listserv with considerable experience in treating HS with LHR were invited to participate in this study. Consensus was defined as an average agreement of 70% or greater. Twelve experts participated in all three modified Delphi survey rounds. Overall, experts believe that hair follicle involvement is central to HS pathogenesis, justifying LHR as a safe and effective treatment for early-stage disease. Additionally, experts agree that early implementation of LHR may prevent disease progression, LHR is likely safe during pregnancy, and LHR can be combined with systemic therapies. The standardization of best practices in LHR for treating HS will reduce variation in care, decrease the risk of adverse events, and promote increased adaptation among clinicians. This will lead to consistent and predictable treatment outcomes for patients with HS receiving LHR treatment.
