Allergic Sensitization Patterns Throughout Childhood in a US Birth Cohort: A Latent Transition Analysis

Abstract

Rationale: Age-specific allergen sensitization patterns may differentially associate with atopic dermatitis (AD) and other allergic conditions including asthma and allergic rhinitis (AR). Methods: Prospectively collected data from a southeast Michigan birth cohort was used to assess longitudinal allergen-specific IgE sensitization patterns. Sensitization patterns at age 2 years (n=589) and 10 years (n=554) were assessed for associations with AD, asthma, and AR. Latent transition analysis was then used to characterize shifting sensitization patterns between age 2 and 10 years. Results: Three latent sensitization patterns at age 2 included: low/no sensitization (LN), milk/egg dominant (ME), and highly sensitized groups (HS). Four patterns at age 10 included: LN, HS, perennial inhalant (PI), and seasonal inhalant with peanut (SIP). Compared to LN at age 2, ME and HS at age 2 were associated with higher odds ratios (OR) for AD at age 2 (ORME=3.22, (1.87, 5.55); ORHS=5.19 (1.96, 13.71) and AD at age 10 (ORME=4.18 (2.13, 8.19); ORHS=3.29 (1.05, 10.26). HS and ME classes at age 2 were significantly associated with increased odds of asthma and AR at 10 years. At age 10, HS, PI, and SIP were significantly associated with concurrent AD risk (ORSIP=6.80 (2.73, 16.93); ORPI=3.52 (2.01, 6.16); ORHS=5.07 (2.60, 9.90)), while HS and PI were significantly associated with concomitant asthma. Latent patterns indicating LN and HS at age 2 had high probability of persisting through age 10, while children with an ME pattern at age 2 had variable transition probabilities. Conclusions: Sensitization patterns throughout childhood change over time and are differentially associated with allergic outcomes.
