Efficacy and Safety of Tapinarof Cream 1% Once Daily Monotherapy in Adults and Children with Atopic Dermatitis: Pooled Results from Adoring 1 and 2

Abstract

Introduction: Tapinarof, a nonsteroidal, topical aryl hydrocarbon receptor agonist, is FDA-approved for atopic dermatitis (AD) down to age 2 years without restrictions on duration, extent, or location of use. Tapinarof is recommended for the management of AD in adults by the American Academy of Dermatology guidelines. Methods: This post hoc analysis of pooled ADORING 1 and 2 data included: Validated Investigator Global Assessment for AD™ (vIGA-AD™) response (vIGA-AD™=0/1 and ≥ 2-grade improvement); ≥ 75% and ≥ 90% improvement in Eczema Area and Severity Index (EASI75, EASI90); and achievement of ≥ 4-point reduction in Peak Pruritus-Numerical Rating Scale (PP-NRS) from baseline, and PP-NRS ≤ 1. Results: 813 patients (80.4% pediatric) were randomized. At baseline, 87.0% had vIGA-AD™=3 (moderate), mean EASI=12.9, and mean PP-NRS=6.7. Significant vIGA-AD™ response (5.8% vs 1.6%, P=0.0315), EASI75 (9.0% vs 3.3%, P=0.0164), and itch improvements (PP-NRS ≥ 4-point reduction, 18.6% vs 11.1%, P=0.0298; PP-NRS ≤ 1, 6.7% vs 0.8%, P=0.0003) were observed with tapinarof versus vehicle at Week 1. At Week 8, primary and secondary endpoints were met with statistical significance with tapinarof versus vehicle (pooled data): vIGA-AD™ response, 45.9% vs 15.9% (P<0.0001); EASI75, 57.4% vs 22.1% (P<0.0001); EASI90, 30.0% vs 8.0% (P<0.0001); and ≥ 4-point reduction in PP-NRS, 55.1% vs 31.2% (P<0.0001), respectively. Significant achievement of no-to-minimal itch (PP-NRS ≤ 1) was observed with tapinarof versus vehicle at Week 8: 32.2% vs 15.7% (P<0.0001). Tapinarof was well tolerated. Conclusion: Tapinarof monotherapy demonstrated early itch improvement and clinically meaningful efficacy across all age groups, and was well tolerated in patients down to age 2 years with AD.
